if in your selection you have files with same result name after renaming, then tool not be able to do actions.
well, I think, he must have option to overwrite files?
Now, you must deselect some files manually, and then call multi-rename tool again

Overwriting may result in unintentional information loss, I don't think it's a good idea.
Perhaps I'll add an option to remove some items from a list directly in this dialog.

Done, now there's a context menu with a single "Remove Item" element.

No. A mean keyboard key. Navigate by arrows, minus (or another key) - to remove from list. Maybe, [space] key.
I think, it will be much faster, than context menu

Good idea, I think Backspace button will be best.

Ok, done, Backspace now deletes an item in this list (if it is focused).
Also, I've added a Ctrl+I hotkey to focus items list.
